Welcome to the Home Care Association of New Hampshire

The Home Care Association of New Hampshire is a non-profit membership organization that enhances the ability of home care agencies to provide the best care possible to New Hampshire residents. We fulfill our mission through education, advocacy, leadership and networking.

We help families and other health care providers by offering information about home health options and resources to connect them to home care agencies in their communities.

Referrals for home care can come from multiple sources including nursing homes, hospital discharge planners, physician offices or community mental health providers. Click here to find home care agencies serving patients in your area. Under our Providers page, you will find information regarding Medicaid, Medicare (including the Homebound and Face-to-Face requirements), links to CMS and other helpful information.

The Home Care Association offers home care agencies on-site and distance education programs; affinity groups for collegial sharing of best practices; regulatory updates; and advocacy in Washington, DC and at the State House. Members can also participate in shared savings programs for products and services, including workers compensation insurance.
